Active Acne, Acne Spots & Acne Scars

  • Acne is a skin condition visible on the face, chest or back in the form of comedones (blackheads and whiteheads), red bumps, nodules and cysts.
  • Acne is caused because of changes in hormone levels, which in turn increases secretion of sebum (oil). This sebum along with dead cells block the pores in the skin. This allows bacteria to multiply, resulting in inflammation and acne. Acne is common among individuals between 15 – 45 years of age.
  • One can keep experiencing acne throughout their adulthood triggered by events that cause changes in the levels of hormones like irregular-periods, menopause etc. The most troubling aspect of acne though is the fact that after one experiences as breakout, the marks, spots and scars can last really long.

Acne can cause two major types of blemishes on your skin which put in simple words are called:

  1. Acne Marks and
  2. Acne Scars

Acne marks or spots are called Post Inflammatory Hyperpigmentation or PIH. These are caused because of the fact that when your skin heals there is often an excess formation of melanin in your skin. This excess melanin leaves the affected spots on your skin much darker than the surrounding area.

Acne scars on the other hand are the changes in the texture or smoothness of your skin. When you skin tries to repair itself after a bout of acne, it uses a protein called collagen. This collagen deposition by your skin is not very even and hence causes a change in the surface of the skin which we call scarring.

Most spots caused by acne tend to fade away in about 3 to 9 months’ time. But some spots and almost all scars need external intervention to heal properly.

Common causes and aggrevators of Acne include:

Hormonal imbalance
Stress
Hot and humid environment
Applying oily/oil-based products
Certain oral medications

It is important to understand that it is far easier to handle active acne than to handle the spots and scars left behind after an uncontrolled breakout. Dr. Sejal recommends that her patients follow the below protocol when they have active acne:
1. Ensure that you never pop a zit, as this is the fastest way to get a scar.
2. Make sure not to fidget with acne as it can tend to spread the bacteria to other parts of your face.
3. Make sure to keep your face free of excess oil or sebum by using a gentle facewash or medicated wipes. Excess sebum creates the perfect conditions for acne causing bacteria to proliferate.
4. Make sure to use the topical medication prescribed by a dermatologist on your active acne.
5. Make sure to use a separate topical medication on the acne that have burst or are in the healing stage. (As prescribed by your dermatologist)
6. Use makeup sparingly and also look for makeup that is non-comedogenic.
If these steps are followed, it reduces the need to handle acne spots and scars substantially

Because of the fact that acne is so widespread there are also a whole lot of myths and bad advice surrounding acne care. Two of the things that I highly
recommend patients to not do are:
1. Never ever use home remedies like vinegar, honey, cinnamon etc. on your acne or spots and scars. These things are of better use in cooking
than on your skin. Best case scenario they will not help you, the worst case scenario they will cause a serious reaction on your skin.
2. Never use over the counter (OTC) creams on the advice of your friendly chemist or pharmacist or friend.

Acne has very poor relation with food intake. Unless you are allergic to a particular food, there is no need to curtail the same from your diet. Please note that relation of foods like mangoes and milk with acne are anecdotal and are not substantiated medically or scientifically.

Home remedies that use clay or anything organic can cause reactions and infections on your skin and should be avoided at all costs.

There is absolutely no concept of ‘body heat’in modern medical science. Acne is purely hormonal in nature and have no relation to any such abstract concepts.

Absolutely not. Irregular Bowel Movements do need to be regularized as in the long term they can cause other complications. However they have no relation to acne. Any such observed relation is purely anecdotal.

Acne is a hormonal condition and hence whenever your body undergoes major events causing hormonal changes like teenage, menstruation, meno pause etc. it can give rise to acne. Also skin hygiene plays a role in controlling acne. People who are siusceptible to acne should keep thei skin dry and devoid of excess oil or sebum.

Stress by itself cannot cause acne. However stress does aggrevate acne and practically all other skin infections.

Yes, acne does subside on its own. But without a proper treatment protocol the acne lesions can become quite severe and wide spread and cause PIH and scarring when they resolve. Hence acne should never be left untreated.

Absolutely not. When you pop pimples it causes an uncontrolled injury to your skin, this will generally result into marks and scarring. Hence it should be absolutely avoided.

1. Acne may start off as whiteheads or blackheads due to clogged skin pores.
2. Acne may evolve into red bumps and boils due to inflammation.
3. As bacterial growth increases, sometimes there is development of pus.
4. Lastly, sometimes cysts are formed due to deep tissue involvement.
