Stretch marks are generally formed when skin expands or shrinks quickly. In this case the connective
tissues under the skin can be damaged and also there are changes in the collagen and
elastin fibres. Stretch marks generally present themselves like lines or striae, where the skin
seems to have been ‘stretched’.
In their early stages of formation, stretch marks are reddish in appearance and as they
mature and settle down, they tend to become whitish in appearance.
Stretch marks commonly develop during pregnancy, puberty or after gaining or losing a lot of
weight. They can also be caused during certain conditions like cushing’s syndrome etc. They
usually appear on the stomach, thighs, upper arms or back.
Dr. Sejal highly recommends a systematic treatment of stretch marks. It generally follows the below protocol:
1. Use of topical stretch marks reduction ointments which try to remodel the skin from the surface. But these have limited efficacy.
2. Chemical Peels also have limited efficacy for treating stretch marks.
3. Use of Microneedling procedures using either dermapen or dermaroller.
4. Use of Co2 Fractional Laser for affecting scars and stretch marks that do not heal with microneedling alone.
5. Use of MNRF – Radio Freuency Micorneedling – for enhanced effect of RF with Microneedling.
Both the procedures of microneedling and CO2 laser are often enhanced with PRP treatment. PRP stands for platelet rich plasma which is derived from the patients blood and is rich in growth factor proteins. These growth factors aid the healing process after skin resurfacing procedures such that the results are often superior.
Stretch marks are best handled by procedures that remodel the collagen and elastin in the skin. The best way to do is to carry out a controlled injury of the skin using certain skin resurfacing techniques. When this controlled injury heals it causes two primary effects:
1. It activates the collagen and elastin formation process in the skin. Collagen and Elastin are the basic proteins which hold the skin cells together and are instrumental in the form the skin takes.
2. This process also leads to neo-vascularization, that is formation of new blood vessels, leading to a much better blood flow and hence healthier skin.
Because of the above two effects when the skin heals, there is a substantial improvement in its texture and in case of any abnormalities like scars and stretch marks the healed skin starts resembling the adjoining area rather than have a different texture or structure.
The results of stretch mark reducing procedures is both permanent and additive. Meaning that with each session of a skin resurfacing procedure there is an improvement in the results and this result is sustained. This is so because once the collagen fibres are restructured in the skin there is no way for it to revert back to an abnormal state unless there is again some factor like injury or a growth spurt to cause stretch marks.
Skin resurfacing procedures are slow to show results as they depend on the body’s natural ability to heal itself. Also the skin resurfacing treatments can only be done slowly and progressively as the treating doctor has to ensure that there is only limited injury to the skin which will aid in smoothening the skin and not harm the skin further.
For seeing visible difference in stretch marks a minimum of 4 sessions is advised by Dr. Sejal. For a complete resolution one may need many more sessions spaced at least one months apart.

Topical creams and ointments have a limited efficacy on stretch marks as they try to remodel the stretch marks from the surface. It is advised to undergo skin resurfacing procedures like microneedling or co2 fractional laser to see the best results. Also topical creams work better when the stretch marks are freshly formed and not set in.
There are two main types of stretch marks:
Striae rubrae: Pink / red stretch marks, which are generally the freshly formed stretch marks.
Striae albae: White stretch marks, these are more mature stretch marks that have set in.
